How to measure respiratory rate
- Let the person rest. Talking, walking, exercise, fever, pain, anxiety, and knowing that breathing is being watched can change the rate.
- Watch the chest or abdomen. One inhale and one exhale—a complete rise and fall—counts as one breath. Do not count both movements separately.
- Count for 60 seconds when possible. A full minute avoids multiplication and is especially useful when breathing is irregular. A 30-second count is a quicker estimate.
- Record more than the number. Note whether the person was resting or active and whether breathing looked regular, shallow, noisy, or difficult. This tool measures rate only.

Why observation time matters
With a 30-second count, every single breath changes the estimate by 2 breaths/min. With a 15-second count, every breath changes it by 4 breaths/min.

A commonly used broad orientation band for a resting adult is about 12–20 breaths per minute. Published sources differ slightly—for example, MedlinePlus lists 12–18 for the average healthy resting adult—so the number is not a diagnosis or a universal cutoff.

Safety and medical limits
A rate inside a reference band does not prove that breathing is safe. Work of breathing, colour, alertness, chest pain, wheezing or other noise, and a sudden change from the person's normal pattern may be more important than the number.
