This equation estimates albumin-adjusted total calcium. It does not calculate or measure ionized calcium and does not determine whether a result is normal.
Do not use this estimate alone to diagnose a calcium disorder or start, stop, or change treatment. Albumin correction formulas can misclassify low, normal, and high calcium status. Their performance varies with the patient population, albumin and calcium assays, kidney function, critical illness, pH, phosphate, and other factors.
Use the method and reference interval supplied by the laboratory. When the distinction is clinically important—especially with symptoms, markedly abnormal results, severe hypoalbuminemia, critical illness, or kidney disease—a clinician may use a directly measured ionized calcium result and other tests rather than this estimate.
These are the common simplified Payne equations. The adjustment uses 4.0 g/dL (40 g/L) as the reference albumin concentration.

The equations are rounded unit adaptations, so results converted between systems can differ by a very small amount. This calculator uses 1 mg/dL calcium = 0.2495 mmol/L for display conversion and applies the equation matching the units you entered.

A 2025 cross-sectional study of 22,658 patients found that unadjusted total calcium agreed with ionized calcium better overall than the original or simplified Payne adjustments. Agreement was poorest when albumin was low—the situation in which correction formulas are commonly used. This does not make every correction useless, but it means the result should be treated as a limited arithmetic estimate rather than “true calcium.”
Corrected calcium, also called albumin-adjusted calcium, is a calculated estimate intended to account for the effect of albumin on measured total calcium. It remains an estimate of total calcium and is not a direct measurement of ionized calcium.

In a large 2025 comparison, unadjusted total calcium agreed with ionized calcium better overall than commonly used adjustment formulas. Corrected calcium should not replace measured ionized calcium when an accurate classification will affect care.
The arithmetic can be performed, but the common correction was mainly adopted for low albumin and may be unreliable outside a laboratory-validated setting. Above 4.0 g/dL or 40 g/L albumin, the equation subtracts from measured total calcium and this page displays an additional caution.
